Step 3: Grief Coaching Agreement Form

After completing the New Client Information Form (above), please read and sign the Grief Coaching Agreement Form.

This document contains important information about my professional services and business policies. When you sign this document, it will represent an agreement between you, the client and Julie Lynn Ashley, Grief Support Services and Education, LLC, whereby Julie Lynn Ashley will provide Coaching Services to you focusing on non-clinical issues. It is very important that you understand the concepts discussed below. Take your time reading it and discuss any questions you have with Julie Lynn.

Electronic payment must be made the day of the session only, not in advance. If a cancellation needs to be made even up to 48 hours ahead of time, this way you have not paid for a session you will not use. There are no refunds, but replacement sessions can be arranged if there is an emergency on the part of the client or the coach.
